Malakoplakia is an uncommon chronic inflammatory disease usually affecting the urogenital tract, mostly urinary bladder followed by other organ systems like kidney, prostate and colon etc. Malakoplakia of testis has been reported rarely and presents as a diagnostic challenge. The lesion occurs mainly in middle-aged men, appearing clinically as epididymo-orchitis. Here we describe a rare case of Malakoplakia of testis in a 75 year old male who arrived at hospital with the complaint of painful and swollen testis. The histological examination plays a crucial role in achieving an accurate diagnosis.
